# Do I have to fill in IC, Passport, Marital Status, Date of Birth?

This column is **only for individual employers** (personal name) who **do not have a valid SSM registration**.  

Employers operating as a **Company (Berhad, Sendirian Berhad, Sole Proprietor, Partnership)** with a **valid SSM registration** **should NOT** fill in the following columns:
* **Identification Card (IC) Number**  
* **Passport Number**  
* **Marital Status**  
* **Date of Birth**
