# I need to change/update employer or worker information.

Please provide the required **company, employer, or worker information** along with **supporting documents**. Our team at **eInsurans** will assist you in verification or cancellation (if necessary).  

Below are the **supporting documents** required for updates:  

### (A) Company
1. Policy Number  
2. Insurance Company Purchased *(Lonpac / Generali / Tokio Marine)*  
3. Copies of all **SSM Company Registration Documents**  

### (B) Employer (Individual / Sole Proprietor)
1. Policy Number  
2. Insurance Company Purchased *(Lonpac / Generali / Tokio Marine)*  
3. Copy of **Identity Card (IC)**  

### (C) Worker
1. Policy Number  
2. Insurance Company Purchased *(Lonpac / Generali / Tokio Marine)*  
3. Copy of **current work permit**  
4. Copy of **passport**  
5. Copy of **Special Pass (SP)** if applicable  

> **Additional Information:** If a worker has updated their passport or permit, you are required to update the worker’s information with **JIM, FOMEMA, and PERKESO / SOCSO** accordingly.
