  1. Government Hospital Admission (Cashless) - Simply present the worker’s passport at the hospital for validity checking. Once the SPIKPA validity is confirmed, the hospital will handle the arrangements with the insurer's third-party provider.


 Note: Occasionally, the hospital may request a deposit payment due to the medical condition or specific hospital procedure requirements.

You may later submit a claim for reimbursement, as long as it is within the coverage benefit and amount limit



  1. Important Information About Government Hospital -
  • Government Hospital refers to any non-corporatized Malaysian Government Hospital (Note: Semi-government institutions like PPUM, UKM, UiTM, and private hospitals are not eligible for SPIKPA coverage).

  1. Manual Claims Submission

If a deposit is paid or for any other manual claim, please submit the following documents:

  • Completed Insurance Company Claim Form

Download the claim form here

  • Copy of Worker’s Passport & Work Permit
  • Original Medical Report (Request a formal copy from the hospital or download it from the hospital website)
  • Original Medical Bill/Receipt

Important: Kindly allow 14 to 30 working days for claim processing after we receive the completed forms. Please ensure all documents are correctly filled out to avoid delays.



  1. How to Request Medical Reports:
  • You may download the medical report form from the respective hospital.

 For example: If admitted to Hospital Kuala Lumpur, you can download the medical report application form from their official website.



  • Upon discharge, promptly request the medical report from the attending doctor or hospital. This may take a few weeks depending on hospital or doctor availability.



  1. SPIKPA does not cover:
  • Emergency outpatient treatment
  • Death
  • Accidents due to fight, unlawful activities, or criminal actions
  • Total Permanent Disability (TPD) (Note: TPD claims can be made through SOCSO under the Employment Injury Scheme)
