How to obtain medical claim report/form

Upon discharge, promptly request the medical report from the attending doctor or hospital.
The processing time may take a few weeks, depending on the hospital’s or doctor’s availability.
Example: If admitted to Hospital Kuala Lumpur, you can download the Medical Report Application Form directly from their official website.
