What Is the Foreign Worker Hospitalisation and Surgical Insurance Scheme (SKHPPA / SPIKPA)?
The Foreign Worker Hospitalisation and Surgical Insurance Scheme (SKHPPA / SPIKPA) is a mandatory insurance scheme in Malaysia that provides hospitalisation and surgical coverage for foreign workers.
This insurance applies when a foreign worker is admitted to a non-corporatised Malaysian government hospital (KKM) due to:
- Accident, or
- Illness
SKHPPA / SPIKPA is required as part of the foreign worker permit (ePLKS) renewal process and is recognised by the Immigration Department of Malaysia (JIM).
Coverage Period
- 12 months coverage per policy period
The coverage period must align with the worker’s permit validity and is typically purchased during permit renewal via FWCMS.
What Does SKHPPA / SPIKPA Cover?
SKHPPA / SPIKPA provides in-patient medical coverage at eligible Malaysian government hospitals, including:
- Daily Hospital Room & Board
(subject to policy limits and maximum number of days)
- Intensive Care Unit (ICU) charges
- Hospital supplies and services
- Operating theatre charges
- Surgical fees
(excluding organ transplants)
- Anaesthetist fees
- In-hospital medical specialist visits
- In-hospital consultation fees
- Ambulance fees (where applicable under the scheme)

- Semi-Government Hospitals
- Private Hospitals or Clinics
