Do I have to fill in IC, Passport, Marital Status, Date of Birth?
This column is only for individual employers (personal name) who do not have a valid SSM registration.
Employers operating as a Company (Berhad, Sendirian Berhad, Sole Proprietor, Partnership) with a valid SSM registration should NOT fill in the following columns:
- Identification Card (IC) Number
- Passport Number
- Marital Status
- Date of Birth
